Phantom Fury was the 2nd Battle of Fallujah. Part of the travesty was the fact the US already took the city but turned it over to questionable Iraqi authority and subsequently had to take it again.

Yes, the Campaign is explicit about what happened during Vigilant Resolve. And, we've interviewed participants of this operation, too.
