Civilian Shooting Penalty Idea

I had this thought about the penalty of what should happen when the player shoots a civilian when reading other comments and thought what if, instead of resulting in an instant mission failure for shooting a civilian, you get 3 chances?
To elaborate, if the player "accidentally" shoots 3 civilians, they get a '1 minute' or '30 seconds' cut scene that's unskippable, and depicts some sort of guilt or mental dissatisfaction with the player's character actions that stops the gameplay or restarts the level/mission.
If the player "intentionally" shoots 3 civilians, they get a different '1 minute' or '30 seconds' cut scene (unskippable), that maybe depicts dishonourable discharge or relatable punishment, and instead of allowing the player to replay the mission or campaign, have their campaign/scenario playthrough reset, a type of permadeath situation.
To address the accidental or intentional, I'm thinking if enemies are within a 3 metre radius of a civilian when the civilian is shot would be classed as accidental, any further would be intentional.
