Thanks for all the comments on this, everyone. We identified and fixed one potential cause of this with 0.1.4.2, but there's still some more investigating for us here. This has escalated to one of our Known Issues.

This is correct - AI will absolutely open doors.
As for players closing doors, we've passed this suggestion to the team. I'll add your notes to the list!

Team Leads can already open the map and place a pin for the entire fireteam to see.

Great question. Without giving away too much under the hood: Spawns are determined when the map is rolled. We do not infinitely spawn AI or move them to try and flank players. If you're commenting about AI spawning behind you, it's actually an enemy that has pushed through your line and legitimately...
